Formulas Used

Gross Annual Cost = Session Rate × Sessions/Month × 12

Copay Plan Annual OOP:

  • Phase 1 (pre-deductible): Pay full session rate until remaining deductible is met
  • Phase 2 (post-deductible): Pay copay per session
  • Annual OOP = min(Phase1 + Phase2, OOP Max − OOP Already Spent)

Coinsurance Plan Annual OOP:

  • Phase 1 (pre-deductible): Pay full session rate until remaining deductible is met
  • Phase 2 (post-deductible): Pay Session Rate × Coinsurance% per session
  • Annual OOP = min(Phase1 + Phase2, OOP Max − OOP Already Spent)

Effective Annual Cost = Annual OOP − min(Annual HSA/FSA, Annual OOP)

Income Ratio = (Effective Annual Cost ÷ Annual Income) × 100

Effective Cost per Session = Effective Annual Cost ÷ Sessions per Year

  • Session rates based on 2024 U.S. averages: $100–$300/session (Psychology Today, 2024).
  • Deductible phase assumes the therapist's billed rate equals the session rate entered (i.e., in-network allowed amount).
  • HSA/FSA funds are applied dollar-for-dollar against out-of-pocket costs; tax savings are not modeled (vary by bracket).
  • Affordability benchmarks (3–5% of income) are derived from general financial wellness guidelines; no single universal standard exists.
  • The Mental Health Parity and Addiction Equity Act (MHPAEA) requires most insurers to cover mental health services comparably to medical/surgical benefits.
  • EAP sessions, Medicaid, and CHIP coverage are not modeled here — contact your HR department or state Medicaid office for details.
  • Calculations assume all sessions occur within the same plan year and that the therapist is in-network.
